Why You're Exhausted in Your 40s (And Why More Sleep Isn't Fixing It)
It shows up around 2pm. Or it never lifts at all, and you move through the whole day with a low hum of tired underneath everything you're doing.
So you go to bed earlier. You cut the evening screens, you try the magnesium, you get a solid eight hours.
And you wake up tired anyway.
Sleep Was Never the Thing Driving It
That's the part that trips women up, and it's the reason so many stop looking. If more sleep were the answer, more sleep would have worked by now.
What I see most often is that fatigue in midlife isn't a sleep problem wearing a disguise. It's an energy production problem. Your body is either not making enough usable energy, not absorbing what it needs to make it, or spending it faster than it can replace it.
Sleep is one input. It matters. But it can't fix a supply issue.
The other explanation you've probably been handed is age. You're in your forties, this is what forties feel like, everyone's tired.
Being tired at 45 is common. Common is not the same as normal, and it isn't the same as permanent. Exhaustion that doesn't respond to rest is your body flagging something, and there are usually only a handful of things it's flagging.
Your Forties Change the Math
The reason this hits now, and not at 32, comes down to a few shifts happening at the same time.
Estrogen and progesterone start fluctuating in perimenopause, sometimes years before cycles change in any obvious way. Both of those hormones influence how your body handles blood sugar, how deeply you sleep, and how your thyroid signals get used.
Stomach acid production also tends to decline with age, which quietly affects how much protein and how many minerals you actually extract from good food.
Add a decade or two of accumulated stress load, and you get a body running on thinner reserves than it used to have, right when the demands on it are highest.
None of that is a discipline problem. It's a physiology shift, and physiology shifts respond to targeted support.
Five Things I Look For When a Woman Tells Me She's Exhausted
When someone comes to me with fatigue that won't lift, these are the patterns I'm looking for in her intake and her labs. Rarely is it just one.
Her Blood Sugar Is Swinging More Than It Used To
The tell is timing. Fine in the morning, wrecked by mid-afternoon. Ravenous two hours after eating. Waking at 3am and not falling back asleep. Irritable when a meal runs late.
Insulin sensitivity often declines through perimenopause, so meals that used to hold you now don't. The crash isn't a willpower failure at 3pm. It's what happens after a breakfast that was mostly carbohydrate.
What tends to help: protein early and consistently (aim for a real amount at breakfast, not a splash of milk in coffee), fat and fiber alongside carbohydrates rather than on their own, and eating on a rhythm your body can predict.
She's Eating Well and Still Not Absorbing It
This is the one most often missed, and it's why digestion is where I start even when the complaint is energy.
You can eat an excellent diet and still run low on the nutrients energy production depends on. B12, magnesium, and iron all require functioning digestion to get from your plate into your cells.
Bloating, irregular stools, reflux, or feeling heavy after meals alongside the fatigue is a meaningful pattern. So is the absence of obvious digestive symptoms, honestly, because absorption issues don't always announce themselves.

Her Thyroid Isn't Getting the Support It Needs
Fatigue, cold hands and feet, thinning hair, constipation, stubborn weight, low mood. That cluster shows up constantly.
Standard bloodwork often checks TSH alone, which can look acceptable while the fuller picture tells a different story. The panel I use includes TSH, free T3, free T4, reverse T3, and thyroid antibodies, so we're seeing whether hormone is being made, converted, and actually used.
The reference ranges matter as much as the panel. Standard ranges are built to flag disease, and they're wide. Functional ranges are narrower, which is why a result that came back technically normal can still point toward nutrition and lifestyle support that's worth putting in place.
Diagnosis and medication decisions belong with your physician, and if something in your results points that direction, that's exactly where I'll send you. What I can do alongside that is address the nutrient status thyroid function depends on. Selenium, zinc, iodine, and iron all play a role in converting thyroid hormone into its active form, and every one of them depends on absorption.
Inflammation Is Quietly Spending Her Reserves
Low-grade inflammation is metabolically expensive. Your body is running an immune response in the background, and that energy comes from the same pool as everything else.
Markers like hs-CRP, ferritin, and white blood cell patterns can point toward it on a blood chemistry panel, though blood work alone won't tell us where it's coming from. That's usually where stool testing comes in, because it looks at what's happening in the gut rather than the downstream effect of it.
It rarely feels like inflammation, which is why it goes so long without being investigated. The signs of gut inflammation are easier to recognize once you know what you're looking at.
Her Red Blood Cells Aren't Carrying What They Should
Red blood cells deliver oxygen. Not enough oxygen delivery means fatigue that shows up as breathlessness on stairs you used to take easily, or a heaviness in your legs during ordinary activity.
Hemoglobin can sit in the normal range while cell size and iron storage tell you something is off. Heavy or unpredictable perimenopausal bleeding makes this more likely, and it's frequently overlooked because the periods themselves get chalked up to normal midlife change.
A full iron panel (ferritin, serum iron, TIBC, transferrin saturation) alongside a complete blood count gives a much clearer picture than hemoglobin on its own. Those markers are part of the blood chemistry panel I use, and I look at them against functional ranges to guide what we do nutritionally.
Please don't start iron on your own in the meantime. Iron accumulates in the body, and supplementing without knowing your status can do real damage. This is one where testing first is not optional.
Where This Leaves You
Notice how much of this comes back to the same few places. Digestion affecting nutrient status. Nutrient status affecting thyroid. Blood sugar affecting sleep and hormones. Inflammation affecting all of it.
Your symptoms aren't unrelated. They just haven't been looked at together.
That's also why fixing one thing in isolation rarely holds. You can eat more protein and still be tired if you're not absorbing it. You can support your thyroid nutritionally and stay stuck if inflammation keeps interfering.
The starting point isn't another supplement or another rule. It's figuring out which of these is actually running the show in your body.
